Athletic Participation Forms

In order to participate in SHC athletics in any capacity (e.g. pre-season conditioning/weight room sessions, tryouts, practice, post-season conditioning/weight room sessions, open facilities), all student-athletes are required to have a current PPE on file and enroll on Appryse to complete the online Athletic Participation Forms.

This is required of all frosh regardless of athletic participation (see Class of 2022 web page).

Apprise is a web-based system to track medical history, emergency contact information, and mandated CIF/SHC athletic forms for students. As parents, you will be able to enroll your child and enter all information online as well as upload the Preparticipation Physical Exam (PPE) form. Information can be updated throughout the year as it changes. A new feature allows up to two parents/guardians to have equal access to the same account and to receive all the same alerts and messages. We are asking parents to enroll their child—students should not enroll themselves. There is information intended for both parents and students and require an electronic signature by both.

If your son/daughter participated in SHC athletics during the 2017-18 school year, they have an existing account. The information you entered last school year will be there. You will just have to review and update when submitting. If they have a current PPE on file, it will automatically be transferred over to the new enrollment.
