We look forward to your contributions to the SHC community! This page serves as a resource to you and your family as you prepare for the 2017-18 school year. Please note that after September 1, 2017, the information provided here will transition to the Class of 2021 Schoology Group.

Immunization and physical form mailing sent on June 2

All families received a blue immunization form, a yellow form describing the immunization policy and a return envelope in this mailing. Parents: Please complete the form based on the immunizations your child received. You are welcome to transpose the records your doctor provided you to the blue form or you may ask your doctor for a copy of your child’s immunization record and attach it to the blue form. If you choose this option, please complete the TOP portion of the blue form with your child’s name and other requested information. As well, you are welcome to upload the immunization form and email it to:

All incoming freshman are required to enroll in Appryse, regardless of whether a student intends to join an athletic team. The deadline for freshman to enroll in Appryse is July 28, 2017. They will not be allowed to participate in SHC athletics at any capacity until they do, including preseason conditioning/weightlifting sessions.

Frosh Orientation

One School, One Book 2017 

Our One School, One Book for 2017 is The Soul of an Octopus. All SHC students are expected to read the OSOB selection, chosen this year by the Science Department; parents and guardians are invited to read as well! Please click on the link for more information.

Course Textbooks

Please click the first link for a full list of textbooks for all courses. Students are expected to start the school year with all of their books already purchased. Families may purchase the books from any site such as eCampus, Amazon, and MBS, the online bookstore with whom SHC works. If you order your books early, a used book option is usually available on the MBS website.

Frosh Welcome Morning

Thank you for joining us! Click here to see slides from the morning’s presentations. If your student was not able to attend, please contact the Admissions office to schedule a make-up math placement test.

Language Other Than English Choice Form (Due Tuesday, April 25)

SHC’s Languages Other Than English (LOTE) Department offers five languages for study: American Sign Language (ASL), French, Japanese, Mandarin and Spanish. Students who respond by Tuesday, April 25 at 3 pm will receive the highest consideration for their first-choice language. Indicate your LOTE choice here.

LOTE students who indicated on their online choice form that they are native speakers in their first choice language or who have already completed formal study in their first choice language are required to take the LOTE Placement Test on Saturday, April 29, after their Math Assessment. If your student has an unavoidable conflict with the LOTE Placement Test, please email LOTE Department Chair Gloria Huerta.

Student & Family Information
(Due Saturday, April 29)

Complete your student and family demographic/emergency contact information by Saturday, April 29. All incoming families are required to complete this process. You will receive the Student & Family Emergency Contact/Handbook Agreement form in our July mailing where you’ll have an opportunity to confirm all of the information you supplied to SHC.

Jumpstart Registration
(Due June 5)

Jumpstart introduces students entering ninth grade to SHC’s active, collaborative and multidimensional learning environment. Each session includes effective study skills, SHC learning resources and educational technology, our Wellness Curriculum, and time in the Fitness Center. All incoming ninth graders are required to attend one session, and must attend the full session.

NOTE: For the educational technology session, students should bring the device they will use during the 2017-18 school year. Please view our minimum systems specifications and Bring Your Own Device (BYOD) FAQs to assist with this.

There is limited enrollment per session, please be sure to register early to ensure your preferred date.

Addtl Jumpstart Info:
On the first day of each session, we’ll meet in the Student Life Center (1055 Ellis Street). Please proceed through the glass doors, go downstairs, and look for the Jumpstart check-in sign.

For the Fitness/Health session: please arrive dressed in appropriate attire (t-shirt, shorts, tennis shoes). You may also want to bring a towel, water bottle, a healthy snack, headphones and a music player! Food and drinks other than water are not allowed in the Fitness Center.

For your Academics/Technology session: All rising ninth grade students are required to have the device they will use during the 2017-2018 school year on campus during the BYOD orientation portion of Jumpstart. Please refer to the minimum systems specifications and FAQ documents on the Class of 2021 webpage to assist with your purchasing decision.

Athletics Pre-participation Physical Exam Form & Appryse

SHC is pleased to announce the use of Appryse Athletic Participation Forms Registration for the 2017-18 school year.

What is Appryse?
Appryse is a web-based replacement to the paper-based Athletic Participation Forms system previously used to track medical history, contact information, and Pre-participation Physical Exam (PPE) forms. Parents are able to enter all information online, including uploading or faxing the PPE form directly to Appryse. It also allows you to electronically submit Emergency Information and electronically sign the following required forms: Policies and Procedures, Awareness of Injury Risk and Consent to Treat, Concussion Parent/Athlete Information, Awareness of Sports Injury Risk, and Sudden Cardiac Arrest Information. We feel it will make this year much easier for parents, coaches, and most importantly, increase the safety of our students.

A few features:

  • You will no longer need to bring a hardcopy or send school physical forms.
  • The PPE form will have a bar code unique to your child.
  • Appryse will contact parents to alert when their child’s physical is close to expiring.
  • In the event of a medical emergency, athletic trainers and coaches will use a system to automatically notify you of an emergency via phone, email, and text message.

Enrolling in Appryse
All incoming freshman are required to enroll in Appryse by July 28, 2017, regardless of whether a student intends to join an athletic team. They will not be allowed to participate in SHC athletics at any capacity until they do. Do not wait until the last minute, as this will affect the student’s ability to participate in SHC Athletics.

Information regarding your son or daughter’s medical history should be updated throughout the year as needed.

Important notes before registering:

  • Enrolling in Appryse will allow you to download a blank 2017-18 SHC Pre-participation Physical Exam (PPE) form, which will include your child’s name and a barcode
  • We will not accept hard copies of PPE forms for incoming freshman.
  • We will not accept hard copies of PPE forms past June 30 for current 10th-12th graders.
  • Please have your physician use the approved Appryse SHC PPE form that has your son or daughter’s unique bar code.
  • Please make sure the Appryse SHC PPE form is properly completed, signed and dated or it will not be accepted
  • Only forms completed by an MD or DO will be accepted. We will not accept forms completed by other health care professionals such as registered nurses (RN), nurse practitioners (NP), physician assistants (PA), and chiropractors (DC).
  • The parent should either fax the Appryse SHC PPE form to the number at the top right of the physical form, or scan and upload it directly to Appryse.
  • Do NOT upload or fax a different physical form, as this will not be recognized by the Appryse system and will NOT be received.
  • Do not include attachments such as immunization forms.
  • Only the SINGLE page Appryse form can be faxed or uploaded.
  • Questions or problems during enrollment should be directed to Appryse. Click the “Contact Support” button on the Appryse website.

Boys and Girls Cross Country:
  • All levels:  Wednesday 8/16 at Kezar Stadium from 3:30-5:30pm
  • Freshmen:  Monday 8/14 at SHC Practice Field (On Eddy Street at Gough Street)

Girls Golf:

  • Freshman: Monday 8/14 at Presidio Driving Range from 2:30pm- 4:00pm.
Girls Tennis:
  • JV/Freshmen:  Monday 8/14 at SF State Tennis Courts from 3:30-5:30pm
Girls Volleyball:
  • JV/Freshmen:  Monday 8/14 at SHC Pavilion from 4:307:30pm

Schoology and PowerSchool Login Information

Parents will be given login usernames and password instructions after the Frosh Parent meeting in August.
